These Terms of Use govern your access to and use of fivestarrefer.com, its forms, content, referral services, employment-connection resources, partner applications, advocate opportunities, communications, and related features.

The website and Five Star Referral Network™ are owned and operated by SS Eisenhauer Enterprises LLC. By accessing the website, submitting information, creating an account, or using a Five Star service, you agree to these Terms of Use and the Privacy Policy.

If you do not agree, do not submit information or use the website’s services.

2. About Five Star Referral Network™

Five Star Referral Network™ provides referral coordination, relationship-based introductions, employment connections, community-support routing, service-provider connections, and network-development opportunities.

Five Star generally serves as a connector and coordinator. Unless expressly stated in a separate written agreement, Five Star does not directly provide the professional, licensed, regulated, employment, housing, financial, medical, legal, counseling, recovery, transportation, childcare, contracting, or other services offered by independent providers.

3. Ownership and Organizational Relationships

Five Star Referral Network™ and Five Star Family Support Alliance™ are owned and operated by SS Eisenhauer Enterprises LLC.

RGV Road to Recovery IA is a separate organization. RGV may collaborate with Five Star on approved community-support initiatives, administer eligible project funding, and receive its calculated participation share from the Five Star Family Support Alliance™ Pool under a written agreement.

This collaboration does not give RGV Road to Recovery IA ownership, management authority, operational control, voting rights, or any equity interest in Five Star Referral Network™, Five Star Family Support Alliance™, or SS Eisenhauer Enterprises LLC.

4. Referral and Coordination Services

Five Star may collect information about a person’s needs and use that information to identify potentially relevant providers, employers, advocates, organizations, resources, or opportunities.

A referral or introduction is not an endorsement, warranty, certification, or guarantee. Users are responsible for evaluating a provider’s qualifications, licensing, insurance, experience, pricing, availability, suitability, safety, reputation, and contract terms before accepting services.

Five Star may decline, delay, redirect, close, or discontinue a request when an appropriate connection is unavailable, information is incomplete, the request falls outside active service categories, or continuing would create legal, operational, ethical, or safety concerns.

5. Independent Providers and Employers

Service providers, employers, advocates, directors, participating organizations, and other network members generally operate independently. They are not employees, agents, representatives, joint venturers, or legal partners of SS Eisenhauer Enterprises LLC solely because they participate in the network.

Five Star does not control an independent provider’s work, pricing, scheduling, hiring, employment decisions, contractual terms, professional judgment, service quality, regulatory compliance, or results.

Any agreement for employment, products, professional services, housing, financing, or other assistance is between the user and the applicable independent party unless a separate written agreement expressly states otherwise.

6. Employment Connections

Five Star may connect job seekers and employers, but it does not guarantee an interview, job offer, employment duration, compensation level, employee performance, background-check result, work eligibility, or hiring outcome.

Employers remain responsible for lawful recruiting, screening, interviewing, hiring, classification, compensation, workplace safety, tax reporting, and employment practices. Job seekers are responsible for providing truthful and accurate information about their identity, experience, qualifications, availability, and work authorization.

7. Professional and Regulated Services

Information provided through the website is general information and referral assistance. It is not legal, medical, psychiatric, financial, tax, insurance, therapeutic, housing, employment, or other professional advice.

Users should obtain advice directly from an appropriately licensed or qualified professional. Five Star does not establish a professional-client relationship between a user and SS Eisenhauer Enterprises LLC.

8. Emergencies and Crisis Situations

Do not use Five Star forms, email, messaging, or referral services for emergencies. If someone is in immediate danger or experiencing a medical emergency, call 911. For suicide or mental-health crisis support in the United States, call or text 988.

9. Eligibility and Accurate Information

You must be at least 18 years old to independently submit an application, enter a participation agreement, or request services through the website. Information concerning a minor must be submitted by, or with legally sufficient authorization from, a parent or legal guardian when required.

You agree to provide accurate, current, and complete information. You may not impersonate another person, submit information without proper authorization, misrepresent qualifications, create fraudulent referrals, manipulate compensation, or use the network for unlawful purposes.

10. Forms, Applications, and User Submissions

Submission of a form does not guarantee acceptance, approval, participation, assistance, employment, partnership, compensation, exclusivity, or a particular response time.

You retain ownership of original material you submit. You grant SS Eisenhauer Enterprises LLC permission to use, store, reproduce, review, route, and disclose submitted information as reasonably necessary to process your request, administer the network, comply with law, prevent fraud, and perform the purposes described in the Privacy Policy and applicable consent language.

Do not submit confidential professional records, medical details, financial account credentials, Social Security numbers, or other highly sensitive information unless the relevant form expressly requests it through an authorized process.

11. Advocate, Partner, and Network Participation

Advocate, director, employer, provider, alliance, and other network participation may require a separate written agreement, approval, verification, training, payment, or continuing compliance with Five Star policies.

Website content does not replace the applicable participation agreement. If these Terms conflict with a signed participation agreement, the signed agreement controls for that relationship.

Five Star may approve, deny, suspend, restrict, or terminate participation based on network needs, qualifications, performance, misconduct, nonpayment, legal risk, policy violations, inaccurate information, or other legitimate business reasons, subject to any controlling written agreement.

12. Compensation and Material Relationships

SS Eisenhauer Enterprises LLC may receive referral fees, placement fees, service fees, advertising revenue, participation fees, commissions, or other compensation from businesses, employers, service providers, and participating organizations.

Advocates, directors, alliance participants, and other approved parties may receive compensation under the then-current written Five Star compensation structure or an applicable signed agreement.

The Five Star Family Support Alliance™ Pool receives ten percent of qualified collected network revenue. Eligible Alliance participants, including RGV Road to Recovery IA when active and approved, may receive a calculated share of that pool. The amount depends on the applicable participation formula and number of eligible participants for the distribution period.

Compensation does not guarantee that a particular provider will be recommended, selected, retained, or successful. Five Star may modify its compensation structure prospectively, subject to controlling written agreements and applicable law.

Referral or compensation arrangements involving regulated professions will apply only when permitted by applicable law and professional rules.

13. Fees and Payments

Any fee, commission, subscription, participation charge, placement fee, or payment obligation must be disclosed through the applicable page, checkout process, invoice, compensation schedule, or separate written agreement.

Users are responsible for authorized charges, applicable taxes, and accurate payment information. Refunds, cancellations, chargebacks, and recurring payments are governed by the specific offer or agreement under which the payment was made.

14. Intellectual Property

The website, Five Star Referral Network™ name, Five Star Family Support Alliance™ name, logos, designs, written content, referral systems, forms, workflows, training, databases, software configurations, and related materials are owned by or licensed to SS Eisenhauer Enterprises LLC.

You may use publicly available website content for ordinary personal and informational purposes. You may not reproduce, modify, sell, license, scrape, republish, distribute, reverse engineer, imitate, register, or commercially exploit protected content, branding, systems, or data without written permission.

15. Acceptable Use

You may not use the website or network to:

• Break any applicable law or regulation.
• Submit false, misleading, fraudulent, or unauthorized information.
• Harass, threaten, exploit, discriminate against, or endanger another person.
• Transmit malware, harmful code, spam, or unauthorized advertising.
• Interfere with website security, forms, accounts, databases, or operations.
• Harvest contact information or network data without authorization.
• Circumvent referral tracking, payment obligations, or network relationships.

By providing contact information and requesting communication, you authorize Five Star to respond through the permitted channels you select. Marketing communications require any consent mandated by applicable law and may be discontinued through the provided opt-out method.

17. Third-Party Websites and Services

The website may link to independent providers, employers, payment processors, platforms, resources, and other third-party websites. Those parties maintain their own terms, privacy practices, security, pricing, and responsibilities.

SS Eisenhauer Enterprises LLC is not responsible for third-party websites, content, products, services, availability, security, or conduct merely because a link or referral is provided.

18. Disclaimer of Warranties

To the fullest extent permitted by law, the website and referral services are provided on an “as is” and “as available” basis. SS Eisenhauer Enterprises LLC disclaims warranties not expressly provided in a signed agreement, including implied warranties of merchantability, fitness for a particular purpose, title, and noninfringement.

Five Star does not warrant uninterrupted access, error-free operation, service availability, provider performance, successful placement, employment, financial results, or any particular outcome.

19. Limitation of Liability

To the fullest extent permitted by law, SS Eisenhauer Enterprises LLC and its owners, officers, personnel, contractors, and affiliates will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from use of the website, a referral, an independent provider, an employer, a third-party service, lost opportunity, lost data, or interrupted access.

Nothing in these Terms excludes liability that cannot legally be excluded.

20. Indemnification

To the extent permitted by law, you agree to defend and indemnify SS Eisenhauer Enterprises LLC from third-party claims, losses, liabilities, and reasonable costs arising from your unlawful conduct, fraudulent submission, material violation of these Terms, infringement of another person’s rights, or unauthorized use of the website, network, data, or branding.

21. Suspension and Termination

Five Star may restrict or terminate access to the website or network when reasonably necessary to address suspected fraud, security risk, unlawful activity, misuse, inaccurate information, contractual breach, nonpayment, circumvention, or harm to users, providers, participating organizations, or the network.

22. Governing Law

These Terms are governed by the laws of the State of Iowa, without regard to conflict-of-law principles. Any legal proceeding must be brought in a state or federal court with jurisdiction in Iowa, unless a controlling written agreement requires another dispute-resolution process.

23. Changes to These Terms

SS Eisenhauer Enterprises LLC may update these Terms prospectively. The effective date at the top of this page will be revised when material changes are published. Continued use after an updated version becomes effective constitutes acceptance where permitted by law.

24. Severability and Entire Understanding

If a provision is unenforceable, it will be limited to the minimum extent necessary, and the remaining provisions will continue in effect. These Terms and the Privacy Policy govern ordinary website use, but a signed agreement controls when it specifically governs a participant, provider, employer, advocate, project, or transaction.
